I've merged what I can from this series to reduce the churn.

I expect there's still some more discussion, things to figure out on the
other patches, but please keep the 'series' count going to v6 for the
next iteration, with a fresh cover letter.

It might be a good opportunity to start posting the ov8858 driver to
linux-media.

That will at least give reviewers an opportunity to start highlighting
what needs to be updated for integration (and linux-media always takes a
long time to get feedback, but I'll try to help there too).
